1. DATES:
a. The first day that a student is physically present in your
class is the date that goes
on the LEIS Sheet. A
Student Entry Date must be the same on all of these forms.
You cannot give a student a
goal and date it the second day of the class. That
student will show up as
not having a goal for that one day.
b. The date on which you drop a student from
the roster must match the date used on
the Outcomes Form. A student cannot
accomplish a goal after they are dropped
from the roll. The date for his/her achievements or separation
will be the last date
of the semester in which he/she was
enrolled. If a student has at least one
achievement, it is not necessary to have a separation reason.
2. All students must complete all applicable
items on the Student Profile Sheet.
3. Every student must have at least one goal
on the data form.
4. It is extremely important that goals are
appropriate and attainable by a student during the
report year. Many
examples show up that are not reasonable. For example, a student
who
took a reading placement test and scored a grade level cannot have a
goal “Learn to
Read (Nonreader)." A prisoner cannot have a goal of “Get a Job” or
“Get a Better Job." A
person born in the United States cannot have a
goal of “U.S. Citizenship."
Social Security Numbers
and/or Student ID Numbers are very important.
We need the student’s social security number on every
form that is submitted for that
student. This is needed to ensure that none of the forms are
lost or misplaced. Preference
is to use Student ID numbers
as opposed to Social Security Numbers. However, if
student has not been issued
a Student ID Number, Social Security Numbers must be
indicated. It is
imperative to have an appropriate ID Number on each student form.
5. Many students are without outcome information.
When there is no information that
shows an achievement or a separation reason,
the reports that go to Raleigh and
Washington, DC count the
student as “Separation Reason Unknown." This is not a
desirable reason for a student to no longer
be in our class. It implies that no effort
was made to find out why the
student is not attending or to get the student to return
to class.

It is easier to fill them out correctly the
first time rather than returning them back to the instructor for corrections.
