COMPLETING THE LEIS FORMS ---
WHAT IS THE IMPORTANCE OF EACH SHEET IN THE
PROCESS?
Contact Program Coordinator for the most
updated forms.
At
the end of each semester, instructors compile and check the LEIS sheets to
ensure the LEIS forms have been completed accurately. LEIS sheets are
due in the Basic Skills Compliance and Reporting Office 15 days after the
end of each semester.
LEIS Student Maintenance Sheet
#1
It
is imperative that a Student Maintenance Sheet is filled out for each
student at the beginning of each semester and submitted as a package along
with the other LEIS forms. This
information is recorded in the Demographics Screen (XNCA). The information is confidential and all
areas of the form must be completed to be successfully processed. Missing information will generate
computer errors.
LEIS Profile Sheet – Sheet #2
This
data information sheet is needed on every new and returning student.
The date on this sheet must be the first day that the student is in your
class and should match the enrollment date on the registration form and on
the class roster. Every student must have at least one documented
goal. We suggest that you help each student set both short and long
term goals. This information is mandatory. All other
information applicable to the student must be checked for statistical
reporting purposes.
LEIS Testing and Progress – Sheet
#3a & 3b
This
sheet must be filled out on all new students and on students who have been
re-tested and have changed program placement. In most cases, the date
on this sheet will be the same date as the one on the Profile sheet.
After indicating type of placement, the date of the test, and the code
number for the test (codes are found on an additional sheet), it is
necessary to place a code for the component of the test.
LEIS Student Outcomes – Sheet #4
Achievements
and separations are reported on this sheet. When a student leaves the
program or transfers to another class, you will need to fill out this
sheet. The date shown for achievements or separations must match the
last day the student was on the class roster, not the last day of the
quarter. It is not necessary to indicate a separation date for Adult High School students. Be sure
to check all applicable information.
