Personnel Files |

Guidelines |
 |
A. The Human
Resources Department will maintain personnel files for current and past employees. The
Associate Vice President for
Human Resources is the designated custodian of such files. |
 |
B. The following
items must be submitted by or for new full-time employees in order for personnel files to
be complete: |
 |
1. A completed CPCC
employment application (which includes required selective service information). |
 |
2.
Official transcripts from
the college(s) attended (the employee must request that the colleges send transcripts to
CPCC) and, if necessary, evidence of required certification
or
competency. |

3.
A completed "Interviewed Applicant Recommendation" form. |
 |
4. A completed salary information form. |
 |
5.
A signed "General Terms of Conditional Employment" form, if applicable. |
 |
C. The following
items must be submitted by or for new part-time employees in order for personnel files to
be complete: |
 |
1. A
completed CPCC employment application (which includes required selective
service information). |
 |
2.
Official transcripts from the college(s) attended (the employee must
request that the colleges send transcripts to CPCC) and, if necessary,
evidence of required certifications or competency. |
 |
3.
A
completed part-time employment agreement. |
 |
D. Credentials must be kept current and complete in the Human Resources
Department. For
full-time and part-time faculty, credentials and evidence of competency
must be kept current and complete in the office of the Assistant to the
Vice President for Learning.
Advanced degrees must be earned, reported, and documented by official
transcripts. |

E. Annual additions
to the file include copies of contracts or salary notices and evaluation reports. |
 |
F. The file may
also include copies of letters of commendation, materials related to job actions, and
other appropriate materials. |
